Radio chatter, in car banter and background satire are the parts that suffer most, because they are rarely the thing you are looking at.
This is not new for the series. According to reporting on the store listings, GTA 5 in 2013 and Red Dead Redemption 2 in 2018 shipped with essentially the same arrangement: one English voice track, a set of localized subtitle and interface languages around it.
For players whose language is on the list, the impact is manageable. For players whose language is not, the game is effectively an English language product with a second language crutch at best.
